question about condoms

so, i was thinking, and i know that some ways people mess up condoms is incorrect application, and holes, etc etc, but what are other ways that can cause pregnancy/issues? im curious to know so i dont make the same mistakes.

We go over some of the common errors folks make in condom use in a few of our articles about condoms: Condom Basics: A User's Manual & All the Barriers! All the Time!. Most errors in condom use are related to improper use or storage that makes breakage or slipping off more likely.
